How menopause affects women's heart health
Learn how menopause affects heart health, your cardiovascular risk, symptoms to watch for and when to seek expert advice.

If you're going through the menopause and starting to wonder whether the changes to your body are affecting your heart, the honest answer is that yes, they very likely are. Menopause itself isn't a disease, but the hormonal shifts that come with it, particularly the drop in oestrogen, have a genuine and measurable effect on your cardiovascular risk. Blood pressure tends to rise. Cholesterol profiles worsen. Weight often redistributes towards the middle. Sleep is disrupted. And the natural protection that oestrogen offered your blood vessels for the previous three or four decades quietly falls away. The result is that a woman's risk of heart disease climbs significantly in the years around and after menopause, catching up with, and eventually matching, the risk faced by men of the same age.
The good news is that this isn't inevitable, and it isn't something to be alarmed by. Menopause is genuinely a window of opportunity. In my experience caring for women across Kingston-upon-Thames and South West London, patients who take proactive steps during perimenopause and the years after typically see far better long-term outcomes than those who wait until symptoms force the issue. From working with women in this age group, I'd estimate that roughly six out of ten of my new consultations from women in their late 40s and 50s relate directly or indirectly to changes that started during the menopause transition. Understanding what's happening, and knowing where to focus your attention, is the difference between reacting to disease later and preventing it now.
Why Menopause Changes Your Cardiovascular Risk
For most of your reproductive years, your body produces oestrogen in significant amounts. Oestrogen doesn't just regulate the reproductive system. It also has a protective effect on the cardiovascular system. It helps keep the inner lining of blood vessels flexible, supports a healthier cholesterol profile, and dampens some of the inflammatory processes that drive atherosclerosis (the gradual build-up of fatty plaque in the arteries).
Around perimenopause, oestrogen levels start to fluctuate. After menopause, they drop to consistently low levels and stay there. That single change removes the natural cardiovascular protection you've had, and it triggers a cascade of secondary changes that together increase heart disease risk over time.
In my experience, the patients who benefit most are those who understand this early. Perimenopause is a genuine window of opportunity to shape the next 30 or 40 years of cardiovascular health, and the interventions during this window are far more effective than trying to reverse damage that's already been done.
What Actually Changes in Your Body
Several things shift during the menopause transition, and each of them affects the heart in a different way.
Cholesterol profile. LDL cholesterol (the "bad" one that drives plaque formation) tends to rise. HDL cholesterol (the "good" one that helps clear plaque) can fall. Total cholesterol often creeps up. These changes typically become measurable in the year or two before your final period and continue into the years after.
Blood pressure. Around 75 per cent of women over 60 have high blood pressure. Some of that is age. A lot of it relates to menopause. Hot flushes and night sweats are increasingly recognised as linked to higher blood pressure and worse cardiovascular outcomes, and treating the underlying vasomotor symptoms often improves the picture.
Body composition. Even without weight gain, body fat tends to redistribute to the abdomen. Visceral fat (the fat around your internal organs) is metabolically active in a way that drives inflammation, insulin resistance and cardiovascular risk. Many women notice their waist measurement climbing even when the scales haven't moved much.
Insulin sensitivity. Post-menopausal changes make type 2 diabetes more likely, particularly in women who were already at risk. This matters because diabetes accelerates atherosclerosis and is one of the strongest drivers of heart attacks and strokes.
Sleep. Hot flushes, night sweats and mood changes disrupt sleep during and after menopause. Poor sleep raises blood pressure, worsens blood sugar control, and drives comfort-eating patterns that push cardiovascular risk in the wrong direction.
Mood and stress. Depression and anxiety during the menopause transition are linked to a higher long-term cardiovascular risk. This isn't about "stress causing heart attacks" in a simplistic sense. It's about the sustained physiological effects of low mood, poor sleep and chronic stress on blood pressure, inflammation and behaviour.
In our experience, small, consistent changes in each of these areas work better than trying to overhaul everything at once. Consistency over perfection, as I often say in clinic.
Conditions That Become More Common After Menopause
Several cardiovascular conditions become significantly more likely in the years after menopause.
Coronary heart disease. The main driver of heart attacks in women. The rate rises steadily in the decade after menopause and becomes one of the leading causes of death in women in the UK.
High blood pressure. By 60, the majority of women have it, and left uncontrolled it drives heart failure, strokes and kidney disease.
Atrial fibrillation. Post-menopausal women are more likely to develop atrial fibrillation, a fast irregular heart rhythm, and women with atrial fibrillation have a higher stroke risk than men with the same condition.
Heart failure with preserved ejection fraction (HFpEF). A form of heart failure in which the heart doesn't fill properly, particularly common in post-menopausal women with a history of high blood pressure or diabetes. It often presents quietly with fatigue, breathlessness and exercise intolerance.
Stroke. Risk rises significantly post-menopause, both from atherosclerosis and from atrial fibrillation.
Type 2 diabetes. Not a heart condition per se, but a major driver of heart disease, and one that becomes far more common in the years after menopause.
Recognising these patterns matters because it changes what we should be looking for during check-ups, and it lowers the threshold for investigating symptoms that might otherwise be dismissed as "just the menopause".
Symptoms to Take Seriously (Not Dismiss as Menopause)
This is genuinely the area where I see the most preventable harm. Because menopause causes so many symptoms in its own right (fatigue, palpitations, sleep disruption, mood changes, hot flushes, brain fog), it's very easy to attribute cardiac symptoms to menopause and miss them.
The symptoms that most commonly get missed include:
Unusual, persistent fatigue that isn't explained by sleep disruption alone
Breathlessness on exertion or when lying flat
New chest discomfort, particularly one that has a relationship to effort or emotional stress
Palpitations that come with dizziness, breathlessness or reduced exercise tolerance
Jaw, neck, upper back or upper stomach discomfort that can't be otherwise explained
Ankle or leg swelling that doesn't settle
Fainting or near-fainting
Any of these deserve a proper cardiac assessment rather than being reassured as menopause-related. This is particularly important because heart symptoms in women are often missed even outside of the menopause context, and adding menopause into the picture makes it easier still for cardiac causes to be overlooked.
In my experience, a proper cardiology consultation works better than repeated GP reassurance in this context because the assessment can look specifically for the cardiac patterns that fit with post-menopausal physiology, rather than starting from the assumption that everything is hormonal.
The Lifestyle Levers That Matter Most
The reassuring reality is that most of what drives cardiovascular risk after menopause is modifiable, and the changes that help are the ones you can genuinely sustain. The general framework for looking after your heart after 50 applies here as well, but with a few emphases particularly relevant to menopause.
Know your numbers. Blood pressure, cholesterol profile, HbA1c (blood sugar) and waist measurement are the four to track. Have them checked at the start of perimenopause and again every one to two years thereafter, so trends can be spotted early.
Move every day, in two different ways. Aerobic activity such as brisk walking, cycling or swimming for around 150 minutes a week improves cholesterol, blood pressure and mood. Resistance training two or three times a week protects muscle mass, insulin sensitivity and bone density. Both together work far better than either alone in this age group.
Eat in a way you can sustain. A Mediterranean-style diet, with plenty of vegetables, whole grains, oily fish, extra virgin olive oil and nuts, is one of the best-evidenced dietary approaches for cardiovascular health. Adequate protein at each meal becomes especially important during and after menopause, because it protects the muscle mass that supports metabolism and mobility.
Protect your sleep. Address night sweats, sleep apnoea (which is under-diagnosed in women), and evening habits that disrupt sleep. Better sleep genuinely improves blood pressure, blood sugar and stress.
Manage stress deliberately. Managing stress is just as important as diet or exercise. During menopause, this is not optional. Building deliberate recovery time into your week isn't a luxury. It's part of the treatment.
Don't smoke. Smoking dramatically increases cardiovascular risk in women, and post-menopausal smokers have an even steeper risk curve. Stopping at any age helps, and the benefits start within weeks.
Watch the alcohol. Alcohol tolerance often changes with menopause, and its effects on blood pressure, sleep and weight become more pronounced. Keeping well within the recommended weekly limits, with several alcohol-free days, is one of the most sustainable changes women make.
HRT and Heart Health, A Balanced View
The relationship between hormone replacement therapy (HRT) and heart health has been misunderstood for years, partly because of older studies that were widely publicised at the time. The current picture, based on more recent evidence, is more nuanced.
For women who start HRT within around 10 years of their menopause or under the age of 60, and who don't have significant existing cardiovascular disease, HRT appears to have a neutral to potentially small favourable effect on cardiovascular risk. Started later, in women well past menopause or with existing significant heart disease, the risk-benefit balance shifts, particularly for oral oestrogen, which is associated with a higher risk of blood clots and stroke.
Modern transdermal (skin patch or gel) HRT appears to have a better safety profile than older oral preparations, particularly for clot risk. That said, HRT is not a heart disease prevention treatment. It's a treatment for menopausal symptoms with a cardiovascular risk profile that needs to be considered individually.
In my experience, the best conversations about HRT and heart health involve both your GP or menopause specialist and, where relevant, a cardiologist. Personal history, family history, cardiovascular risk factors and symptom severity all matter, and the decision should be individualised rather than generic.
When to Seek a Proper Cardiac Assessment
Some situations warrant a proper cardiology review during or after menopause rather than watching and waiting.
Any symptoms suggestive of cardiac disease (chest discomfort, breathlessness, palpitations, dizziness, blackouts, ankle swelling or reduced exercise tolerance) deserve to be investigated, not dismissed as menopause. So does a strong family history of heart disease, particularly premature heart attack in a parent or sibling. Rising blood pressure or cholesterol that isn't responding to lifestyle change, or a stubbornly high cardiovascular risk score on standard calculators, are also good reasons to seek specialist input.
A proactive baseline assessment during perimenopause or in the first few years after menopause is genuinely worth considering. In my experience, women who have a thorough baseline cardiac review during this window are far better placed to spot subtle changes later and to make informed decisions about lifestyle change, HRT, and (where indicated) medication such as statins or blood pressure treatment.
Conclusion
Menopause changes your cardiovascular risk, but it doesn't have to define it. The years around and after menopause are one of the most important windows in a woman's life for shaping long-term heart health, and the levers that make the biggest difference (movement, diet, sleep, stress, alcohol, smoking, blood pressure control and cholesterol management) are all in your hands. Combined with the right medical input where needed, and a low threshold for having symptoms properly assessed, most women can go on to enjoy healthy, active decades ahead.
